ADSL MIGRATION ORDER FORM

Use this form to request migration of your existing ADSL connection to Calloo ADSL. If you require a new ADSL connection then please use this form.

Note: Payment is not made using this form, you will be sent an invoice which can be paid on-line using a secure site.

Prices are per month including VAT.

2. Please enter the ADSL line phone number and your MAC. The phone line number MUST be the number which you currently use for your ADSL connection. The MAC will be supplied to you by your existing provider.

Should you require a router you may order one now, normal price £34.00 but only £10.00 when ordered with a new connection.

6. Payment Payment is normally made by credit card. Once your order has been processed you will be sent an invoice to the contact e-mail address given above. This will contain a link to allow you to make secure payment by credit/debit card. Physical provision of ADSL will not proceed until this invoice has been paid. Total order value is £ (Includes ADSL subscription for first month, additional hardware, VAT and delivery). Should you wish to pay by an alternative method please contact us before submitting a request for provision of ADSL.
